What is "Trigger finger"?
Trigger finger is narrowing of the tendon pulley that surrounds the flexor tendon that bends the finger. As the finger moves, the tendons that move it move within a set of pulleys that attach the tendons to the bones. When a thickening of the tendons is formed that envelops the tendons or, alternatively, the narrowing of the pulleys in which they pass, the same events occur as "caught finger", jump finger, stuck finger, sore finger, and so on.
In fact, the trigger or "jumping finger" is a common occurrence. The typical condition is considerable sensitivity at the bouncy finger base and a finger that is stuck or does not align - mainly in the morning.
Trigger finger treatment is divided into conservative therapy that includes occupational therapy with splints, physical therapy which is usually aided by topical ultrasound and paraffin treatment as well as oral therapy for anti-inflammatory pills.
You can do an injection for the trigger finger base - both cortisone and a combination of cortisone and hyaluronic acid or hyaluronic acid only. I use an injection technique I learned in the United States during which a gentle first-wheel perforation is performed, so I can perform the rest of the injection with no pain. Hopefully, higher success rates are attained with this technique. Sometimes when surgery fails to deliver the goods (there is still sensitivity and movement restriction) I use local injection therapy.
Trigger finger release surgery is done under local anesthesia and during that the tendons are released in the pulley. The chances of success are high.
It is very important to start moving your fingers on the day of surgery.
My protocol is as follows:
- Hand raised for about 24 hours
- Move all fingers at least 10 times every hour
- Painkillers as needed
- From the day after surgery - Ensure full finger movements 10 times an hour - for at least two days
- If heat exceeds 38 degrees or there is discharge from the operation wound - contact Dr. Oron immediately.
